How do I refer my dentist to be a provider for Diversified Dental Services, Inc.? [Member]

Please use the provider referral page to request an information packet. The packet we send will include the fee schedule, current provider directory, a list of our current clients or subscribers and all the necessary paperwork agreement, credentialing form, etc. to become a participating provider. If you have any questions about the materials, please feel free to contact us or call the phone number in the packet. We are currently looking for new providers.

Which dentist s can I see? [Member]

You can receive care from any licensed dentist. However, if you see an “in-network” provider from the Diversified Dental Services, Inc. network, you will receive those services at a reduced fee. Using an in-network provider will reduce your out of pocket costs and your dental benefits will last longer.
If you use an out-of-network dentist, they can charge you the difference between what your plan pays and their regular retail fees.

Do I need to notify my insurance which dentist I am seeing? [Member]

Not usually. Dental plans do not normally require that you provide the name of your specific dentist. However, you will want to contact your claims administrator to be certain.

Can I change dentists? [Member]

Yes. Most dental plans will allow you to change dentists at any time. However, your plan may have a limit on the frequency of x-rays and exams, so there may be additional costs to you if you change before the next available exam period. Please contact your claims administrator for details.

What if I want to see a dentist who is out-of-network? [Member]

The majority of the plans affiliated with Diversified Dental Services, Inc. will pay for care from an out-of-network dentist. You will want to verify how that coverage applies by contacting your administrator. It almost always means that the member will pay more of the total cost of care .

How do I get benefit and eligibility information or verify claims status? [Member]

All benefit information is available through the claims administrator. If your benefits are provided by your employer, they have contracted with a claims facility or insurance company to handle benefit and eligibility information. The phone number should be on your dental benefits card.

How do I get out-of-network benefit information? [Member]

You can get specific information about the out-of-network benefits for your dental plan by contacting the claims administrator listed on your benefits card.

How can I refer my provider to become a participating dentist? [Member]

Please send us the provider information name, address, phone number on the provider referral form. We will send the office a packet of information and invite them to become a participating provider. Please remember that they have the option as to whether they participate.

How do I report a problem with a dentist? [Member]

Can the dentist charge me for copies of my records and/or x-rays? [Member]

Yes. For example, Nevada state law NRS 629.061 2 indicates that a dentist may charge a reasonable fee for copies of x-rays. The statute requires that all medical and dental providers retain a set of records for every patient they treat. Consequently, the dentist must retain a set of x-rays. The statute allows the dentist to charge for the cost of the copies.

I’m a State of Nevada Employee/Retiree and I have selected the HMO Medical option, what dental benefits do I have? [Member]

All State of Nevada Employee/Retiree participants have the same dental benefit, regardless of which medical option they choose. The dental benefit is provided by the State of Nevada Self-funded dental plan. Benefit information can be obtained by contacting the claims administrator, Benefit Planners, toll-free at 1-877-963-8232 www.benplan.com or by visiting the PEBP website.

Can I get insurance through Diversified Dental Services, Inc.? [Member]

Diversified Dental Services, Inc. does not provide dental coverage. We serve only as an independent preferred provider network for large employers and insurance carriers. If you are interested in a personal dental plan, please contact a life and health insurance agent in your area.

What does it mean when a General Dentist limits their practice to a certain specialty? [Member] General Dentists are trained to provide all forms of dental treatment. Some General Dentists prefer to focus their practice on a certain area of treatment e.g., Children’s Dentistry, Prosthodontics, Cosmetic Dentistry, etc. . The General Dentist is free to choose the type of service he or she wishes to provide and by so doing, can become more familiar with that area of care. They can still practice all of the other forms of dentistry. However, they have not obtained the specific specialty licensing which would limit them to that area of treatment exclusively. Dentists who have obtained a specialty license can only offer those services that are within their area of expertise.

How do I get benefits and eligibility information? [Provider]

All benefits and eligibility information can be obtained by contacting the claims administrator. Diversified Dental Services, Inc. does not have access to any claims or benefits data. If you are a participating provider, the Clients and Administrators list Subscriber Listing or the Insurance Company list in your Administration Manual will provide the name, address, and phone number of all of the claims administrators for each client we represent.

Where can I get a copy of the fee schedule? [Provider]

For participating providers, the fee schedule is included in your Administration Manual under the “fee schedule” tab. The schedules are applicable for the calendar year. If the schedule that you have does not show the current year in the header, please contact us for an updated version.

How do I get updated materials for my Administration Manual? [Provider]

Administration Manual materials are delivered to your office when you first become a participating provider. After that, you will receive periodic updates to include in your folder s . If you are missing any materials or are not sure if you manuals are current, please contact us.

Do I use the same fee schedule for all of the Diversified Dental Services, Inc. patients? [Provider]

Yes. Diversified Dental Services, Inc. provides a single fee schedule that applies to all of its clients. The only difference will be the actual benefits applicable to that plan.

How do I report a problem with an incorrect claims payment? [Provider]

Most claims payment issues can be resolved by simply calling the insurance administrator. Once you have brought the issue to their attention, the claims office can usually make arrangements for the claim to be reprocessed.
It is recommendable to indicate that you are an "in network provider" and have them enter your practice TIN or SSN to verify that the claim was processed according to the PPO contract in-network . If you are unable to resolve the issue by contacting the administrator directly, please contact us or call the numbers listed below.
Note: Remember to document the administrator phone number, date of call, the name of the person with whom you spoke, and a brief explanation. We will use this information will help with the follow up.

What does the inside and default mean on the Zip Search? [Website] Inside : Most Zip codes are more than one mile wide. The search is based upon the center of the zip code. By selecting this, typically, you only get providers in your zip code.

Default : If you do not change the distance you want to search in, it is set up to automatically choose a 5 mile distance from the center of the searched zip code. If 5 miles doesn't return enough or any results, select broader mileage.
