is designed to give you some insight in to the Diversified Dental Services approach to dental PPOs and
provider contracting. I hope you will find our approach refreshing. We call it "common sense management."
Who Are We?
Diversified Dental Services, Inc. is a Nevada based, privately held dental PPO development and management
company. We manage dental PPOs statewide in Nevada, as well as Mississippi and Tennessee. Unlike many
dental PPOs, we enjoy a positive relationship with our providers and our clients.

Our Goals
- Establish a fee schedule that is responsive to our contracted providers.
- Carefully review the fee schedules annually and make increases where appropriate. (All fee changes are effective on January first of each year).
- Make the PPO/Managed Care dental process as easy as possible for participating providers.
- Act as an advocate for participating providers in claims issue resolution.
Benefits of Participating in the DDS, Inc. Network
- As a contracted provider, your practice information will be listed in the provider directory, which is made available to all covered employees.
- Your office will receive a Dental Administration Manual which contains information about:
- How and where to submit claims
- The employers and insurance companies that we represent
- Participating provider responsibilities and information about the network
- Diversified Dental Services, Inc. will act as your advocate and help answer questions or resolve issues that may arise.
In short, we want to make it as easy as possible for you to provide services to our membership.
How do I become a Provider?
To become a Diversified Dental Services, Inc. in-network provider you must fill out the following forms.
For your convenience we have given you a checklist to help you keep track of what you need to send us.
The PDF file located below must be saved, filled out and then faxed or mailed back to us using the address on the credentialing form.
You must have the completed credentialing packet to us by the 15th of each month for you to be effective the 1st of the following month.
There must be one packet for each provider per office. For example, if you own two offices with two dentists working at both locations at anytime of the week, you will need to send us four credentialing packets.

After becoming a Diverisified Dental Services, Inc. Dentist, you will be required to re-credential at a given interval. You will receive a letter that will notify you when to re-credential. Current Dentist forms are listed below.
